Quote Request Forms:
For quote and rate information for Tall Tree please contact our staff. Tall Tree for 51+ will require underwriting to review the information to determine if they will quote your group or decline to quote. Please note it can take 7 to 10 business days to go through the underwriting process.
Tall Tree will take other carrier RFP forms in place of their forms for quote requests.
Employer-
- You do not have to use this census we will just need the information on the census
Employee-
If the company does not have prior health insurance please contact our staff for the necessary forms they will need to complete.
New Business Submission Forms:
Once the company decides they would like to continue forward with MCA we will give you the custom employer forms for that company.
