Welcome to Stone Hill National!

We are a one stop shop for all of your Group and Individual needs. Please see the forms link to download your appointment applications. The application process takes 7-10 days depending on the carriers. We have both health and ancillary options available for appointment.

 

Select Health is not currently appointing any new agents at this time; however, we can put you under Stone Hill’s appointment for the time being. The differences you will find are 1) commissions will be paid to you from Stone Hill instead of Select Health 2) Stone Hill is listed as the agent of record, however, we still view it as YOUR book of business. Once your Select Health book grows large enough to meet their monthly requirement of $5,000, we can move your book of business under your own appointment.

In the meantime, you will need to fill out an AOR (agent of record) for each new client. Please send these AOR’s to Jamie Dawes (jamied@stonehill.net) for processing.

*Provided Stone Hill has received the commissions for your policy, we will only adjust back commissions up to 6 months from the date of the AOR.

Please note as you are filling out the paperwork, that most of the carriers will send your commissions to Stone Hill National. We will pay out 100% of your commission to you. Please see the Stone Hill Affiliation section for necessary payment forms.

 

We will also need a copy of your current:

• Completed Appointment Licensing Form
• Completed Credit Authorization
• Copy of your current Utah insurance license
• Copy of your current E&O insurance certificate
• Completed W-9 form, including your signature and tax ID number
• Voided Check

• All eight 2016 CMS Certificates, which include:

*Individual Marketplace Course

*Individual Marketplace Exam

*Affordable Care Act & Marketplace Basics Course

*Affordable Care Act & Marketplace Basics Exam

*Individual Marketplace Agreement

*Privacy and Security Standards Course

*Privacy and Security Standards Exam

*Certificate of Completion for the Individual Marketplace Curriculum
